054 -Health Care - Injury or illness reports
Any fatality which occurs at camp or which results from camping activities or any injury or illness which occurs at camp or which results from camping activities and which is attended by a physician, nurse, or person in charge of health care at the camp, and as a result of which the person (1) is sent home, or (2) is admitted to a hospital, or (3) has a clinical report, laboratory analysis, or x-rays performed which result in a positive diagnosis, shall be reported to the Office of Early Childhood within twenty-four hours by telephone by the camp director. The original report form of any fatality, illness or injury reported to the Office of Early Childhood shall be maintained at the camp or sponsoring organization for a minimum of two years.
